There could be any number of reasons and it's not necessarily that whoever the real culprit is even used an IP address that belonged to your friend at some point- The blanket methods they use to determine who to send these letters to are simply incredibly moronic and I wouldn't be surprised they end up sending cease and desist orders to the entire IP block.

So it could be a non secure, open proxy running at the ISP your friend subscribes to, it could be someone from the ISP itself, it could be that whoever sent the cease and desist is a dumbass, and of course, it could be that yuor friand is a dirty liars, who knows?
