C&D letters are forwarded from the ISP; they have to look up who was assigned to send you that letter since that information is not publicly available unless you are on a static ip and your personal information can be traced to something like a whois lookup.

If your "friend" is running a wireless network, it's possible someone nearby hopped on to download something. If he is on cable internet, then it's also possible that someone could've spoofed the mac address of his modem (although I think this would cause denial of service). There's a lot of things it could be. There's also a bit of software trickery you can do with torrents, but if you're looking for a specific answer, you'll have to ask a specific question.
