To get 5 skulls is complete victory. Each level has X number of rift points. Each time an orc gets through the rift, the game subtracts a rift point from your score. If the point total reaches 0, you lose the level. Other factors contribute to the score, such as your total number of combat kills, trap/minion kills, combos, etc. Moreover, each level has a par time for completion. You can go over par time and even reach a rift point score of only 1 and still win the level, but your score will be low, which affects how many "skulls" you earn. The skulls are used to upgrade your traps/minions.
If you get 5 skulls, this means that you completed the level under par time and no orcs made it through the rift. If one orc gets through, you drop to 4 skulls. If you're one second over par time, you drop to 4 skulls.
To 5-skull a level is total victory -- perfection. The only way you can do better is to get more combo points, which puts you higher on the leader board.
If you 5-skull every level in War Mage difficulty, you earn the coveted Ultimate War Mage achievement. I 3-skulled the finale last night, so I have more work to do.
