Please take heed:
Early in the main story, when you get to Weynon Priory, Joffre will tell you to find Martin in Kvatch.
Do this as early as possible, like at level 1. If you wait too long, the level scaling will make the monsters in Kvatch extremely hard to manage. This is about the only serious level-scaling problem in the game, but it's really huge.
