There's a thread about how it's done on the forums, but I can't find it.
Anyhow, I went back to kill Asthmadon, but he was gone. My achievement list says I got the one for killing him in Nightmare co-op. So apparently to get these boss achievements, you can just stroll outside the zone and let a friend do all the work. I was unhappy, but I'll get over it. Now I'm in Act 4, which is rather dull compared to Act 4 in D2.
