Adrenaline has 3 not so obvious effects on the player other than the temporary health and movement speed increase: It lets you revive downed players faster, it prevents your character from being slowed down when hit by a common infected and it disables your melee timer for its duration so you can melee with your gun for as long as the adrenaline lasts. Therefore adrenaline should be saved for when you're going through an endless horde rush or for when everyone gets KOed and you need to urgently revive them.
Another really important item in L4D2 is boomer bile, these are practically required if you want to get through an endless horde unscathed, and definitely mandatory if you want to get through an expert mode endless horde without having to sacrifice half your team.
