In the almost unheard of cases where this happens, it is always the fault of the game.
I should add that most of those rare cases occur when I'm in a game with Necro. This is not to disparage Necro or suggest that his equipment is anything but top-drawer or that his software is anything but totally pristine and lawful-good. I'm just reporting the facts. Necro, I don't know what to say, bro. Have you considered Nortons?
