Possibilities:
1. Bad vidya card
2. Bad DVI or VGA connection (source: card, monitor, or cord)
3. Bad PCIe connection (on card or motherboard)
4. Bad monitor
5. Incorrect BIOS setting
Solution:
Throw away computer. Start over.
