All kinds of things affect detection, including movement, enemy line of sight, light vs. shadow, and how much weight you're carrying. Even when you're standing still, just looking around is considered movement, so you may think you're hidden but you'll get [CAUTION] notices when sneaking just for turning your head. Five additional pounds can get you detected. However, I think the Silent Running perk gets rid of the weight issue.

As I recall, I spend the late/end game in the best combat armor I could find -- Mark 2 or something. By then, my perks were helping quite a bit.

If you're trying to sneak during the early game, metal armor has drawbacks, but if you get detected, it may be worth wearing.

Certain clothing and light armor probably add to Sneak, but since you're fragile starting out, it's a toss-up. The main thing in the Bethesda series is perks and how items of all kinds affect those perks. I don't think Obsidian departs very far from that relationship.

So I guess you do what you can and wear what helps until you get the perks you want. Ultimately, perks are better than anything -- guns, armor, etc. S.P.E.C.I.A.L. attributes like Perception, Agility, and Luck also affect outcomes more than items.
