It has a Sim element. You almost inevitably become the leader of a faction called the Minute Men. You're given quests to go around starting little "settlements" in order to recruit members. You have to provide food, housing, defense, etc. for the NPC settlers, who are rather like Sims. It's possible to ignore this part of the game, or you can get really carried away with it, as I did. If you don't want to babysit NPCs, you can get a mod called
SimSettlements
, which assigns settlement building responsibilities to the NPCs. It's a wonderful accomplishment, but I prefer doing it myself.
