I've just been assuming you have the Reaper of Souls expansion. I did not enjoy the story or the maps in that, but it adds a lot of nice features. Also, you originally couldn't get beyond level 60 unless you had RoS. I have no idea whether they changed that later.
In fact, RoS completely changes the game. If you only have vanilla, stop playing immediately and begin building Fallout 4 settlements.
