Theoretically, it's supposed to match you with people near your own skill, or tell you when opponents are above or beneath your skill. In my case, just about ever game has people way above me, and there doesn't seem to be any way to keep them together.
In other words, the skill-matching feature is just window dressing that doesn't really work. So in the end, there's not much difference between QuakeLive and Q3A.
But at least you get to play with relatively evenly matched people for a little while.
